Isolation Precautions Quick Reference

- Droplet (surgical mask + private room): Influenza, Pertussis, Mumps, Rubella, Meningitis
- Contact (gown + gloves): MRSA, VRE, C. diff, Scabies, Wound infections
- C. diff: Soap and water ONLY β alcohol hand gel does NOT kill spores
- Standard precautions: Apply to ALL clients, ALL body fluids
Insulin Types
- Rapid (Lispro/Aspart): 15 min onset, peak 1β2h, duration 3β5h
- Short (Regular): 30β60 min onset, peak 2β4h, duration 5β8h
- Intermediate (NPH): 1β3h onset, peak 4β12h, duration 16β24h
- Long (Glargine/Detemir): 1β2h onset, NO peak, duration 24h
- Mixing: Clear before cloudy (regular before NPH)

Positioning Quick Guide
- Respiratory distress / dyspnea: High Fowler's (90Β°)
- Post-op (general): Semi-Fowler's (45Β°) unless contraindicated
- Shock / hypotension: Modified Trendelenburg (legs up, head flat)
- Increased ICP: Head of bed 30Β°, head midline, avoid hip flexion
- After lumbar puncture: Flat for 4β8 hours to prevent headache
- Enema / rectal exam: Left lateral (Sims') position
- After total hip replacement: No hip flexion >90Β°, no internal rotation, no adduction
- Spinal anesthesia: Supine for 8β12 hours
- Epistaxis (nosebleed): Lean forward, pinch nose β NOT backward (aspiration risk)

- Not linear β client may move back and forth
- Anger at nurse = displacement (don't take it personally)
